Aboitiz Pitch

WAIVER FORM

The undersigned having applied for the use of the Aboitiz Pitch (The Outlets Multi-Sports Field), hereby agrees and binds himself and the group/ organization/ company he/she represents to the conditions set forth as follows:

1. The use of the facility shall be restricted for the activity applied for. In this regard, The Outlets at Lipa, Lima Land, and AboitizLand, Inc. reserves the right to review, re-examine and/or reconsider request for the use as applied by the sponsor.
2. Clearance/ permit for the use of the facility shall be applied for by the sponsor(s)/ representative on a First-come-first-serve basis cleared by the Reservations In-Charge and The Outlets management.
3. Narcotics, betting/ gambling and carrying of firearms, bladed and deadly weapons are strictly prohibited in the Aboitiz Sports Field premises. Likewise entering the premises under the influence of narcotic/ alcohol is banned.
4. Appropriate sports attire/ gear and paraphernalia as required in the activity must strictly be followed. Loitering is strictly prohibited and proper disposal of garbage is a must.
5. The 102 x 68m playing  field may be downsized by moving the soccer/ disc cones.
6. All entering and exiting vehicles must register with the security personnel, which will include the type, make, color, plate number and driver’s identity. Vehicles must only be parked at the designated parking area.
7. Scheduled activity may be moved depending on the availability of the field. An overtime of a fraction in the agreed applicable number of hours by the sponsoring group/ organization/ company will be charged equivalent to a full hour. Failure to pay the fine will be the basis of future request disapproval.
8. The management require at least twenty-four (24) hours written or electronic notice for any cancellation and or change on bookings and reservations. Otherwise, regular rates and charges shall be applied.
9. Only authorized personnel shall be allowed to operate the equipment/ facilities belonging to Aboitiz Pitch.
10. The management shall be free of any obligation and liability in the event of damage to property or persons within and outside the facility.
11. The sponsoring group/ organization/ company shall be liable for pilferage, breakage or damage to the equipment(s), furniture(s) and fixture(s) due to negligence/ carelessness of any member.
12. The sponsoring group/ organization/ company binds and obligates itself to pay the charges as indicated in the front page of this document, before the scheduled event.
